1.   Go to Formed  (or click on the "Reborn" image at the left to be          directed there)
2.   Click on Sign Up
3.   Enter the Name of your Parish or Organization
4.   From the drop-down menu, select Mary Help of Christians                 5980 University Dr., Parkland
5.   Enter your name and email address
6.   Sign in to Formed to start watching
7.   Once in the Formed Home page, look for Programs
8.   Select Sacraments
9.   Click on the Reborn image
10.  Watch videos  (The Class consists of Episodes 1, 2 & 3 and               "Reborn: The Gift of Godparents")
11.  Call or e-mail Ana.Rocco to request the Questionnaire in              order to get certified for the Class
